Manejo Práctico del Paciente con Glaucoma Avanzado

Abstract

El glaucoma es una enfermedad crónica que debemos evaluar, evolucionar en el tiempo y tratar. El tratamiento previene la mayor pérdida de fibras nerviosas del nervio óptico. En el glaucoma avanzado estas premisas se deben cumplir, pero sin margen de error por el gran daño que el nervio presenta. Los estudios diagnósticos y evolutivos tienen sus características al presentar tan pocas fibras nerviosas. Usaremos los mismos estudios, pero enfocados a estudiar la isla central de visión. Y por supuesto, el tratamiento debe ser más agresivo para poder cumplir con la premisa que el daño no aumente. Tiene una exigencia mayor por la necesidad de presiones intraoculares (PIOs) bajas y constantes. Diversas fallas en el manejo del glaucoma a nivel social hacen que todavía haya personas que se queden ciegas por esta enfermedad. Son fundamentales los controles como el tratamiento para prevenir el mayor daño del nervio y la ceguera.
