Excimer Laser y Queratocono

Abstract

Objetivo: Evaluar las alternativas del excimer laser para el manejo de casos con queratocono. Materiales y Métodos: Nosotros estudiamos 28 casos con queratocono leve, o con sospecha de queratocono, cuyas características se encontraban dentro de los criterios mencionados de inclusión. En la población analizada, a 16 años del procedimiento, la asimetría infero-superior fue de 1.71, con un promedio de edad de 42.5 +/- 3.1 años. El equivalente esférico preparatorio fue de -4.77 +/- 1.43 dioptrias, con component esférico de -4.18 dioptrias y cilíndrico de -1.31 dioptrias. Los resultados postoperatorios fueron de -0.66 +/- 0.39 dioptrias, con esférico de -0.44 dioptrias y cilindro de -0.5 dioptrias. Resultados: La agudeza visual corregida preparatoria fue de 0.84 +/- 0.13 y la postoperatoria de 0.89 +/- 0.21. Todos los casos en el postoperatorio mediato se encontraron dentro de 1 dioptria de corrección, y 18 casos dentro de 0.5 dioptria de corrección. Importante es destacar respecto de la seguridad del procedimiento que todos los casos conservaron su mejor agudeza visual corregida. La agudeza visual no corregida presentó resultados satisfactorios: 100% veían 20/40 o más y 57.1% veían 20/20 o más. Conclusiones: con un criterio cuidadoso de inclusión, el excimer laser es una alternativa válida de tratamiento en pacientes con queratocono.
